When should you contact a college coach about interest in their school and what information should you include? And how do you determine whether you are realistically a better match for a DI, DII, or DIII school?

You should start to contact the coaches probably during the summer or fall (Depending on the school) your junior year. Tel them what club team you will be or, your high school team, your contact info, your academic information and statistics (VERY IMPORTANT), and what tournaments/showcases you will be going to.
And you should ask yourself that. D1 is a job. You are paid (if you get a scholarship) to play lacrosse. You are working out all year round. It is a huge commitment and you most likely won't have much of a college life.
D2 is intense as well. A lot of working out and a huge time commitment on your part.
D3 is a little less intense depending on where you go. The top teams are very competitive and will run their programs like a D1. There are however a lot of teams that academics are first. D3 sports take a bit of a back seat to the education. The NESCAC schools are very very good academic schools and regardless of whether you are an athlete or not, you need to take school seriously.
